Additional information
I noticed this issue on my iPhone (where I hadn’t opened my vault in a while) this morning but everything continued to work as expected on my iPad until this evening, when I got a message saying I should save my edits because my file path was invalid. I checked the file in iCloud using Files and indeed my changes had not been saved. As part of the troubleshooting guide, I turned on restricted mode and my vault disappeared, leading my iPad to have the same issues as my iPhone did this morning.
I noticed problems with other iCloud services today. I restarted my phone, which solved all issues. Restarting my iPad solved the issues there as well.
I’m embarrassed that the solution was turning it off and on again.
