You can’t just write “NA” for every part of the template. That isn’t helpful at all.
In addition to actually filling out the template, I’d add what you have there in a

fenced code block
```
so we can see what’s actually going on. https://help.obsidian.md/syntax#Code+blocks
moved to help section for not following the BR report template.
Above everything in this case I think looking at the source of the document would be helpful.
