QuickAdd multi-line input prompt

What I’ve tried to do - create a QA input choice with a template that has mulitple variables and enabling the multi-line input prompt. I still get a separate prompt for each variable. Cannot find any information about how to effect a multi-line prompt. I’ve seen examples but they don’t say what they did to get them.
